What documents should I bring when I register ?

When applying for a new health card, you must bring three original documents to prove citizenship/immigration status, residency in Ontario and identity.

if you were born in canada you need your birth certificate, 2 pieces of ID, and something to prove where you live (i.e. letter addressed to you)

My doctor said if you have one hold on to it because the new ones expire, where as the red/white ones are good indefinately.
